Life Coaching Agreement
The following represents the Contract between From the Heart Life Solutions Virtual Coaching Service, LLC, and you (the “Client”). The Services contracted shall be that of life coaching, as set out below (From the Heart Life Solutions Virtual Coaching Service, LLC). Please read it carefully and raise any questions or concerns that you may have with me before agreeing to the contract. This agreement will begin upon acceptance by Client and shall remain valid for the duration of the Client-Coach relationship.
How Coaching Works
I invite you to think of Coaching as a step by step process of habit change to accomplish your goals. Together we will look at your life with a new perspective. You will set goals to uncover your strengths and talents. Through your strengths and talents, my coaching process will take you from where you are now to where you want to be. Some people can achieve change for themselves in a short amount of time and some people may need a little more time to achieve and sustain change.
During our coaching conversations, I will be genuine, honest, respectfully outspoken to a point and empathetic. I encourage the Client to respectfully do the same. The success of the coaching sessions depends upon the Client’s commitment and openness to the process. If the Client believes that the coaching process is not working as desired, the Client agrees to openly communicate their concerns immediately to the Life Coach.
Referring a Client to Therapy or Medical Health Care
Please note that life coaching should not be used as a substitute for mental health counseling, psychotherapy, psychoanalysis, mental health care or substance abuse treatment. I will not use life coaching in place of any form of medical diagnosis, medical treatment or medical therapy.
I will make every effort to recognize and understand when a client has mental health issues or medical health needs that are outside the competencies and scope of life coaching. All referrals will be discussed in detail with the client and offered to the client when appropriate. Decisions regarding medical referrals will follow the guideline resources recommended by (ICF) - the International Coach Federation: Hullinger, A. M. and DiGirolamo, J. A. (2018). Coaching Service Communication
All coaching sessions will be conducted by virtual meeting through Zoom, which is a web-based video conferencing tool that can be accessed on a computer, I-pad or smartphone. Connection to Zoom is free and connection can be made by providing an email link or meeting ID plus password to the client. There are times when communication may be conducted by telephone or text message. However, consistent primary communication via virtual meetings and email is highly encouraged.
Confidentiality
From the Heart Life Solutions Virtual Coaching Service, LLC fully protects the privacy and confidentiality of all client communications and records. At no time will From the Heart Real Life Solutions Virtual Coaching Service, LLC voluntarily reveal or disclose the coaching relationship between themselves and their Clients without written permission from the Clients, unless required to do so by law.

- Life Coach agrees to maintain the ethics and standards of behavior established by the International Coaching Federation (ICF). The International Coaching Federation (ICF) is the leading global organization dedicated to advancing the coaching profession by setting high standards, providing independent certification, and building a worldwide network of trained coaching professionals. Policy if Coach Cancels a session
- If the Life Coach cancels an appointment with a client. The client will be notified 24 hours in advance of a scheduled appointment. The client will not be charged for the canceled appointment. The client will have the option of rescheduling the virtual appointment or may receive a full refund for the canceled virtual appointment.
Client Expectations
- Client is solely responsible for creating and carrying out his or her own physical, mental, and emotional well-being, decisions, choices, actions, and results arising out of or resulting from the coaching relationship or interactions with the Coach. As such, the Client agrees that the Coach is not and will not be liable or responsible for any actions or inaction, or for any direct or indirect result of any services provided by the Coach. Client understands coaching is not medical therapy and should not be used as a substitute for medical or mental health therapy if needed, and does not prevent, cure, or treat any mental disorder or medical disease.
- Client further acknowledges that he/she may terminate or discontinue the coaching relationship at any time based on the terms listed within the client/coaching agreement.
- Client acknowledges that coaching is a comprehensive process that may involve different areas of his or her life, including work, finances, health, relationships, education, and recreation. The Client agrees that deciding how to handle these issues, incorporate coaching principles into those areas and implementing choices is exclusively the Client’s responsibility.
- Client acknowledges that coaching does not involve the diagnosis or treatment of mental disorders as defined by the American Psychiatric Association and that coaching is not to be used as a substitute for counseling, psychotherapy, psychoanalysis, mental health care, substance abuse treatment, or other professional advice by legal, medical or other qualified professionals and that it is the Client’s exclusive responsibility to seek such independent professional guidance as needed. If Client is currently under the care of a mental health professional, it is recommended that the Client promptly inform the mental health care provider of the nature and extent of the coaching relationship agreed upon by the Client and the Coach.
- The Client understands that to enhance the coaching relationship, the Client agrees to communicate honestly, be open to feedback and assistance and must create the time and energy to participate fully in the program.
- If Client needs to cancel any appointments, the client agrees that it is the Client's responsibility to notify the Coach 24 hours in advance of the scheduled virtual meeting. The Life Coach reserves the right to bill the Client for a missed appointment if prior notification is not given within 24 hours of the scheduled appointment. Coach will attempt in good faith to reschedule the missed appointment if the client respects and follows the cancellation policy as described.

Discovery Virtual Coaching Session- Available to first-time potential clients who are trying to decide if coaching is a possible option. I offer a 30-minute virtual coaching session to discover what your current goals and challenges are. I explore if or how my guidance can support you in achieving your goals and solving your challenges. Gives you a chance to sample my coaching style to determine if I am a good fit for your needs. Reclaiming Self-love- A Gateway to Building Meaningful Relationships: If you need some extra support to get past the emotional roadblocks, this program is perfect for the client who wants to feel good about self and build healthy connections.

Payment Policy- Based on the payment option you select; coaching fees must be paid prior to each session. Each month you will receive an invoice from my practice letting you know the amount of payment for that month. You can log in to the private client website area, click on the Payments Menu and you will see any outstanding balances due at that time and you can pay the balance due via credit card or debit card through the PayPal system. In the event, the Client fails to pay any amount due prior to a scheduled coaching session, the session will be canceled at the Coach’s discretion.
Additional Sessions- Post completion of a coaching program, the client has an option to graduate if all goals have been achieved. If goals are not fully achieved and the client feels further coaching sessions are needed, the client can choose to re-enroll into the same program and continue virtual coaching sessions until the final goals are achieved. Or the client can choose to enroll in another coaching program package if all goals have been achieved within the current coaching program.
Appointment Sessions
Prior to each appointment, the client will receive by email an appointment reminder containing all information needed to connect virtually through zoom video. The Life Coach and client commit to start and finish each session on time. If the client is more than 10 minutes late for a coaching session, the Life Coach will assume the session is canceled and the client will forfeit the session fee.
Communication in between scheduled virtual appointments
In the event that you would like support on something that comes up in between our meetings, you can post an inquiry to the Client Log and we can have "just in time" coaching via your Private Client Website. Client will receive a response within 24 hours.
Client Appointment Cancellation Policy
I ask that you give 24-hour notice if you need to cancel or change the time of an appointment. I will make every effort as well, to re-schedule sessions which are canceled in a timely manner per policy. If the client has not completed payment prior to the appointment and is more than 10 minutes late for a scheduled appointment or fails to show for a scheduled appointment, a $25 non-cancellation or non-notification fee will be charged in addition to charging the full regular fee for the missed appointment and both fees must be paid prior to scheduling the next appointment. If a client does not cancel appointments in a timely manner on a regular basis, I will need to end our coaching relationship effective immediately.
Money-Back Guarantee
I call it my "participate and get your money back" guarantee. If within the first month of our coaching relationship you show up to the calls and participate fully in the coaching process, you can simply let me know this is not working for you and I will completely refund your money. To honor the coaching agreement and the coaching process, if you sign up and do not show up for your coaching sessions, do not do the follow-up work, etc., then you will not receive a refund of your money.
Termination Process
The Client or the Life Coach has the right to terminate this Agreement based on the terms listed within this agreement. As a courtesy to the coaching agreement, a two-week written notice of cancellation is encouraged. Client agrees to compensate the Life Coach for all coaching services that have been received and for any remaining unpaid fees that include through and on the effective date of termination of the coaching relationship.
Limited Liability
Except as expressly provided in this Agreement, the Life Coach makes no guarantees, representations or warranties of any kind or nature, express or implied with respect to the coaching services negotiated, agreed upon and rendered. In no event shall the Life Coach be liable to the Client for any indirect, consequential, or special damages. Notwithstanding any damages that the Client may incur, the Life Coach’s entire liability under this Agreement, and the Client’s exclusive remedy, shall be limited to the amount actually paid by the Client to the Coach under this Agreement for all coaching services rendered through and including on the termination date. Therefore, the Life Coach is not responsible for the client's transformation or results. The Client is responsible for their own personal outcomes and transformational results based on the work implemented by the Client.
Dispute Resolution
If a dispute arises out of this Agreement that cannot be resolved by mutual consent, the Client and Life Coach agree to attempt to mediate a resolution in good faith within 30 days after notice has been discussed among both the client and the Life Coach.
Entire Agreement
This Contract constitutes the entire agreement between the Client and the Life Coach and the Life Coach shall not be held responsible and shall remain blameless for any adverse or unintentional consequences, including but not limited to financial, personal, employment or other losses, arising out of decisions or actions the Client may make as a result of the Services provided by the Life Coach. The Agreement may not be amended, altered, or supplemented except in writing signed by both the Life Coach and the Client.
